Although relatively small in size—ranging from 15 to 20 centimeters in length and located in the back of the abdominal cavity, directly behind the stomach—this organ carries an immense responsibility.
Medically, the pancreas is classified as both an exocrine and an endocrine gland. This complexity of function makes pancreatic health crucial for determining the overall stability of the body's metabolism.
Anatomy and Location of the Pancreas
Anatomically, the pancreas is shaped like a flattened pear or an elongated tongue. It is divided into several main sections:
- Head (Caput): The widest part located within the C-shaped curve of the duodenum (the first part of the small intestine).
- Neck (Collum): The narrowed segment connecting the head and the body of the pancreas.
- Body (Corpus): The main section that crosses the posterior abdominal wall.
- Tail (Cauda): The thin, tapered tip pointing toward the left near the spleen.
The pancreas connects closely with the main digestive tract via the bile duct and the pancreatic duct (ductus pancreaticus), which empty into the small intestine to deliver digestive enzymes.
The Two Main Functions of the Pancreas
As a dual gland, the pancreas performs two vastly different yet equally essential physiological functions:
1. Exocrine Function (Food Digestion)
The largest portion of pancreatic tissue (approximately 95%) is responsible for producing digestive juices containing vital enzymes. Every day, the pancreas produces about 1.5 to 3 liters of pancreatic juice. These enzymes include:
- Amylase: Breaks down complex carbohydrates (starches) into simple sugars (glucose) for easy absorption by the body.
- Lipase: Works together with liver bile to break down fats into fatty acids and glycerol.
- Proteases (such as trypsin and chymotrypsin): Digest proteins into amino acids ready to be absorbed by the small intestine.
Pancreatic fluid is also alkaline because it contains bicarbonate ions. This alkalinity is vital for neutralizing stomach acid entering the small intestine, allowing digestive enzymes to function optimally without damaging the intestinal lining.
2. Endocrine Function (Blood Sugar Regulation)
The endocrine portion of the pancreas consists of clusters of cells called the Islets of Langerhans. Despite their small mass, these cells secrete hormones directly into the bloodstream to regulate blood glucose levels:
- Insulin: Produced by beta cells, it lowers blood glucose levels by enabling cells to absorb glucose for energy.
- Glucagon: Produced by alpha cells, it raises blood glucose levels by stimulating the liver to release stored glucose when energy is low (during fasting or hypoglycemia).
- Somatostatin and Pancreatic Polypeptide: Supporting hormones that regulate the secretion of other hormones and digestive system activities.
Disorders and Diseases of the Pancreas
When the pancreas malfunctions, the balance of the body's metabolism and digestion is drastically disrupted. Major medical conditions involving the pancreas include:
1. Pancreatitis (Inflammation of the Pancreas)
Pancreatitis is inflammation of the pancreas that can be acute (sudden and severe) or chronic (long-term).
- Acute Pancreatitis: Often triggered by gallstones blocking the pancreatic duct or excessive alcohol consumption. Symptoms include severe upper abdominal pain radiating to the back, nausea, and vomiting.
- Chronic Pancreatitis: Progressive destruction of pancreatic tissue, typically caused by long-term alcoholism or genetic factors, which can lead to the loss of digestive function and insulin production.
2. Type 1 and Type 2 Diabetes Mellitus
- Type 1 Diabetes: An autoimmune disease where the immune system mistakenly attacks and destroys the insulin-producing beta cells in the pancreas, leaving the body unable to produce insulin.
- Type 2 Diabetes: A condition where the pancreas still produces insulin, but the body's cells become resistant to it, or the pancreas fails to produce enough insulin over time.
3. Pancreatic Cancer
Pancreatic cancer is one of the most dangerous malignancies because symptoms are often undetectable in early stages (asymptomatic). Symptoms usually appear only after the tumor grows, such as jaundice (yellowing of the skin and eyes), drastic unexplained weight loss, abdominal pain, and digestive issues.

Through its exocrine function, the pancreas ensures that the body can efficiently digest food and absorb essential nutrients. Through its endocrine function, it maintains stable blood glucose levels so that all cells in the body receive a balanced supply of energy. Maintaining pancreatic health through a nutritious balanced diet, avoiding excessive alcohol consumption, and managing body weight are crucial preventive steps to protect against serious diseases such as pancreatitis and diabetes.
